Following a spinal cord injury, a person may need support across many different areas of life. This can include rehabilitation, personal care, mobility, housing, equipment, work, education, relationships and emotional wellbeing.

 

At Birchwood & Co Case Management, we help the individual and their family understand the available options, decide what needs to happen and coordinate the professionals involved. Our focus is on developing practical support that reflects the person’s priorities and promotes as much choice, control and independence as possible.

What is spinal cord injury case management?

​

Spinal cord injury case management is the assessment, planning and coordination of care and rehabilitation following a spinal cord injury.

​

The case manager looks at the whole picture rather than considering each need separately. They work with the client, their family and the multidisciplinary team to develop an agreed plan and ensure that recommendations lead to practical action.

 

Support may begin while someone is still in hospital or a specialist spinal injury unit. It can then continue through discharge, rehabilitation and the person’s longer-term adjustment to life at home and in their community.

We support people with complete and incomplete spinal cord injuries, including those with paraplegia and tetraplegia, following road traffic collisions, workplace incidents, falls and other traumatic events.

 

How we support people following spinal cord injury

Every spinal cord injury is different. The level of support required will depend on the nature of the injury, the person’s health, home environment, family circumstances and what they want to achieve.

 

Our case managers can:

• Complete an Initial Needs Assessment

• Develop and review an individual rehabilitation plan

• Coordinate discharge planning with hospitals and spinal injury units

• Arrange specialist therapy and clinical assessments

• Establish, recruit and oversee care and rehabilitation teams

• Coordinate wheelchair, seating and posture management assessments

• Support decisions about equipment and assistive technology

• Arrange property assessments and adaptations

• Assist with adapted vehicle assessments

• Support a return to education, employment or community activities

• Monitor progress and respond to changes in need or risk

• Provide clear reports and recommendations to the wider professional team

 

Coordinating the multidisciplinary team

Spinal cord injury rehabilitation often involves several professionals and services. Depending on the individual’s needs, the wider team may include spinal consultants, GPs, specialist nurses, occupational therapists, physiotherapists, psychologists, wheelchair services and care professionals.

 

We maintain communication between those involved, clarify responsibilities and monitor whether agreed actions are being completed. We also liaise with NHS services and specialist spinal units to support continuity between statutory and privately funded provision.

 

This coordinated approach helps reduce duplication, identify gaps and keep rehabilitation focused on the outcomes that matter to the individual.

 

Specialist care alongside case management

Some people living with a spinal cord injury require personal care, rehabilitation support or a complex care package at home.

​

Because Birchwood provides both case management and specialist care services, we can develop and oversee joined-up packages within the same organisation where this is appropriate for the client.

​

Our CQC registration covers Personal Care and Treatment of Disease, Disorder or Injury. This enables care and relevant clinical or therapeutic support to be delivered by appropriately qualified and competent professionals, with suitable governance and oversight.

The client remains at the centre of all decisions, and their care and rehabilitation arrangements are reviewed as their abilities, circumstances and goals develop.

 

Supporting long-term independence

​

Independence does not mean the same thing for everyone. For one person, it may mean returning home safely. For another, it may involve managing their own care team, returning to work, driving again, travelling or taking part in family and community life.

Our role is to understand what independence means to each client and help put the right support in place to make those goals achievable.
